You're losing track of where your characters are
The last reliable position is uncertain; a known journey that cannot fit is a different problem.
A character's movement between places doesn't make sense
A known movement must be possible given its route and interval; uncertain whereabouts should be resolved first.
A location has changed without you meaning it to
The established place itself appears inconsistent, rather than a journey through it being implausible.
You've lost track of an important object
The object's last reliable position or holder is uncertain, rather than a known history contradicting itself.
A character has an object they shouldn't have
A particular person's access to an object is unsupported, rather than its general whereabouts being unknown.
An object's history contradicts itself
The recorded sequence of object states conflicts; a missing location alone is not the same problem.
